第一篇：請(qǐng)假條的格式

請(qǐng)假條的格式

由于某種原因你不能去上學(xué)，或不能參加某項(xiàng)活動(dòng)，就應(yīng)向老師請(qǐng)假。

怎樣寫請(qǐng)假條呢？

請(qǐng)假條一般應(yīng)包括以下幾個(gè)部分：

（1）稱呼。要項(xiàng)格寫，并在后面加冒號(hào)。

（2）正文。要另起一行，空兩格。主要寫明請(qǐng)假的原因和請(qǐng)多長(zhǎng)時(shí)間假。

（3）請(qǐng)假人姓名和日期。寫在正文的左下方。

李老師：

我因要去參加少年宮的大合唱彩排，今天下午的班隊(duì)活動(dòng)不能參加，特此請(qǐng)假。

領(lǐng)導(dǎo)會(huì)在你的請(qǐng)假條上簽字的。一般你們單位肯定規(guī)定：多長(zhǎng)(更多請(qǐng)搜索：Www.)時(shí)間由誰(shuí)來準(zhǔn)假，要找對(duì)人，還有請(qǐng)假期間的待遇問題要協(xié)商好。
